What is Pleural Fluid?

Pleural fluid হলো ফুসফুস (lungs) এবং বুকের দেয়ালের (chest wall) মাঝখানে থাকা একটি স্বাভাবিক lubricating fluid।
তবে, কিছু রোগে এই তরল অস্বাভাবিকভাবে জমে যায়, যাকে বলা হয় Pleural Effusion।

Pleural effusion সাধারণত দেখা যায়—

• Infection

• Tuberculosis

• Cancer

• Pancreatic disease

• Esophageal rupture

• Trauma

What is Amylase?

Amylase একটি digestive enzyme, যা মূলত—

• Pancreas

• Salivary glands

থেকে তৈরি হয়।

এই enzyme সাধারণত—

• Blood

• Urine

এ পরীক্ষা করা হয়।
কিন্তু কিছু special condition-এ pleural fluid-এ amylase বেড়ে যেতে পারে, যা disease diagnosis-এ অত্যন্ত গুরুত্বপূর্ণ clue দেয়।

Why is Pleural Fluid Amylase Test Done?

এই test সাধারণত করা হয় যখন—

• Pleural effusion-এর cause unclear

• Pancreatitis-এর history আছে

• Chest pain + abdominal pain থাকে

• Esophageal rupture suspected

• Malignancy suspected (especially lung cancer)

• Amylase blood test abnormal আসে

অতএব, unexplained pleural effusion-এর ক্ষেত্রে এই test অত্যন্ত গুরুত্বপূর্ণ।

Conditions Associated with High Pleural Fluid Amylase

1️⃣ Acute or Chronic Pancreatitis

Pancreas থেকে enzyme leak হয়ে pleural cavity-তে পৌঁছাতে পারে।

2️⃣ Pancreatic Pseudocyst

Fluid leak হয়ে pleural space-এ জমে যায়।

3️⃣ Esophageal Rupture (Boerhaave Syndrome)

Salivary amylase pleural fluid-এ বেড়ে যায়।

4️⃣ Malignancy

বিশেষ করে—

• Lung cancer

• Ovarian cancer
  এ ক্ষেত্রে amylase elevated হতে পারে।

Normal & Abnormal Amylase Levels

🔹 Normal Pleural Fluid Amylase:

• Serum amylase-এর কাছাকাছি বা কম

🔹 High Pleural Fluid Amylase:

• Serum level-এর চেয়ে বেশি

• 1000 IU/L হলে pancreatic cause strongly suspected

👉 High amylase মানেই pancreatic or esophageal pathology consider করা হয়।

Interpretation of Pleural Fluid Amylase Report

✅ High Amylase Level

যদি pleural fluid-এ amylase বেশি থাকে, তাহলে—

• Pancreatitis likely

• Pancreatic fistula suspect

• Esophageal rupture possibility

• Malignancy evaluate করা হয়

❌ Normal Amylase Level

যদি amylase normal থাকে—

• TB

• Cardiac failure

• Liver disease
  এইসব cause consider করা হয়।

How is Pleural Fluid Collected?

Pleural fluid collect করার process-কে বলা হয় Thoracentesis।

✔ Procedure details:

• Trained physician দ্বারা করা হয়

• Local anesthesia দেওয়া হয়

• Ultrasound guidance ব্যবহার করা হতে পারে

• Sample lab-এ পাঠানো হয়

👉 Procedure সাধারণত safe এবং কম ঝুঁকিপূর্ণ।

Is Any Preparation Needed?

সাধারণত—

• Fasting দরকার নেই

• Blood thinner medication থাকলে doctor-কে জানাতে হবে

• Previous reports সাথে রাখলে সুবিধা হয়

• Helps identify pancreatic cause of pleural effusion

• Differentiates TB vs pancreatic disease

Limitations of the Test

তবে কিছু limitation আছে—

• Alone diagnostic নয়

• Other tests-এর সাথে correlate করতে হয়

• Rare cases-এ false elevation হতে পারে

👉 এজন্যই clinical correlation অত্যন্ত জরুরি।

What Happens After a High Amylase Result?

যদি report abnormal আসে—

• CT scan / MRI advised হতে পারে

• Gastroenterologist consult করা হয়

• Pancreatic disease treat করা হয়

• Cancer workup শুরু হতে পারে

Early diagnosis মানেই complication কম হওয়া।
