What is Synovial Fluid?
Synovial fluid হলো একটি পরিষ্কার, হালকা হলুদ রঙের তরল যা আমাদের joint (সন্ধি বা গাঁট)–এর ভেতরে থাকে।
এই তরলটি joint movement smooth রাখে, friction কমায় এবং cartilage-কে protect করে।

👉 Synovial fluid আমাদের হাঁটু, কাঁধ, কনুই, ankle ইত্যাদি joint-কে ব্যথা ছাড়া নড়াচড়া করতে সাহায্য করে।
What is Synovial Fluid Malignant Cell Test?
Synovial Fluid Malignant Cell Test একটি বিশেষ cytological examination, যেখানে joint fluid-এ cancerous (malignant) cells আছে কি না তা দেখা হয়।
এই test-টি করা হয় মূলত:
Joint swelling এর cause জানা
Cancer spread (metastasis) সন্দেহ হলে
Chronic joint effusion without clear cause
Unexplained pain ও stiffness থাকলে
অর্থাৎ, এই test joint-এ ক্যান্সার সংক্রান্ত involvement আছে কি না তা নির্ণয়ে সাহায্য করে।
Why is This Test Important?
Normally, synovial fluid-এ malignant cells থাকে না।
যদি malignant cells পাওয়া যায়, তাহলে তা একটি serious clinical finding।
This test is important because:
It helps detect joint-related malignancy
Identifies metastatic cancer involvement
Differentiates cancer from infection or inflammation
Guides further imaging and biopsy
তাই, এটি একটি confirmatory and supportive diagnostic test।
When is Synovial Fluid Examined for Malignant Cells?
Doctors এই test recommend করেন যখন:
Joint effusion দীর্ঘদিন ধরে আছে
Pain, swelling treatment-এ improve হচ্ছে না
Known cancer patient-এর joint symptoms দেখা দেয়
Recurrent fluid accumulation হচ্ছে
X-ray বা MRI suspicious finding দেখায়
Therefore, this test is done only when clinically indicated।
Types of Cancer Associated with Malignant Cells in Synovial Fluid
Malignant cells synovial fluid-এ দেখা যেতে পারে:
Metastatic carcinoma (lung, breast, prostate)
Hematological malignancy (leukemia, lymphoma)
Synovial sarcoma (rare)
Bone or soft tissue tumors spreading to joint
However, synovial malignancy is rare but serious।
Normal Result of Synovial Fluid Malignant Cell Test
✅ Normal Result:
No malignant cells seen
This indicates:
No evidence of cancer involvement in the joint
Joint swelling likely due to non-malignant causes like arthritis, infection, or injury
Abnormal Result – Malignant Cells Detected
❌ Abnormal Result:
Presence of malignant / atypical cells
This may suggest:
Metastatic cancer to joint
Primary synovial tumor
Hematologic malignancy involvement
Symptoms may include:
Persistent joint pain
Progressive swelling
Limited joint movement
Weight loss or fatigue (systemic signs)
Such findings require urgent oncological evaluation।
How is Synovial Fluid Collected?
✔ Procedure: Arthrocentesis
✔ Performed under sterile conditions
✔ Local anesthesia used
✔ Fluid sent for cytology examination
The procedure is:
Safe
Minimally invasive
Usually done OPD or hospital setting
Mild pain or discomfort may occur but serious complications are rare।
Tests Done Along with Malignant Cell Examination
To reach a final diagnosis, doctors usually combine this test with:
Synovial fluid cell count
Protein & glucose
LDH
Gram stain & culture
Crystal analysis
Imaging (X-ray, MRI, CT scan)
Sometimes biopsy
Hence, malignant cell detection is part of a complete diagnostic approach।
Limitations of Synovial Fluid Malignant Cell Test
Although very useful, this test has limitations:
Low sensitivity in early disease
Malignant cells may be sparse
False negative possible
Requires expert cytopathologist
Therefore, a negative result does not always rule out cancer।
