What is Pleural Fluid for Protein Test?

Pleural Fluid for Protein Test একটি গুরুত্বপূর্ণ diagnostic test, যা pleural fluid-এ থাকা protein level পরিমাপ করার জন্য করা হয়।
এই test মূলত pleural effusion-এর কারণ নির্ণয় করতে সাহায্য করে।

What is Pleural Fluid?

Pleural fluid হলো ফুসফুস (lungs) এবং বুকের দেয়ালের (chest wall) মাঝখানে থাকা অল্প পরিমাণ স্বাভাবিক তরল।
Normally এটি ফুসফুসকে smooth movement করতে সাহায্য করে।

However, যখন কোনো কারণে এই তরল বেশি পরিমাণে জমে যায়, তখন তাকে বলা হয় Pleural Effusion।

Pleural effusion হতে পারে—

• Heart failure

• Liver disease

• Kidney disease

• Infection (TB, pneumonia)

• Cancer

• Autoimmune diseases

Why is Pleural Fluid Protein Test Done?

এই test করা হয় কারণ—

• Pleural effusion-এর type বোঝা দরকার

• Disease diagnosis দ্রুত করতে

• Treatment plan ঠিক করার জন্য

• Cancer বা TB সন্দেহ হলে

Most importantly, এই test দিয়ে pleural effusion কে ভাগ করা হয়—

• Transudative pleural effusion

• Exudative pleural effusion

Role of Protein in Pleural Fluid

Protein level pleural fluid-এ বাড়া বা কমা disease process-এর উপর নির্ভর করে।

👉 Low protein → systemic problem
👉 High protein → local inflammation or disease

Therefore, pleural fluid protein test একটি key investigation।

Normal Range of Pleural Fluid Protein

Generally,

• Pleural fluid protein < 3.0 g/dL → suggests Transudate

• Pleural fluid protein > 3.0 g/dL → suggests Exudate

However, final diagnosis always depends on clinical correlation and other tests.

Transudative vs Exudative Pleural Effusion

🔹 Transudative Pleural Effusion

Protein কম থাকে কারণ inflammation নেই।

Common causes:

• Heart failure

• Liver cirrhosis

• Nephrotic syndrome

👉 এখানে problem শরীরের fluid balance-এর কারণে।

🔹 Exudative Pleural Effusion

Protein বেশি থাকে কারণ inflammation বা tissue damage হয়।

Common causes:

• Tuberculosis (TB)

• Pneumonia

• Malignancy (Cancer)

• Pulmonary embolism

👉 এখানে pleura নিজেই diseased।

Light’s Criteria and Protein Test

Light’s Criteria হলো pleural effusion classify করার gold standard।

Protein-এর role:

• Pleural fluid protein / Serum protein > 0.5 → Exudate

• Pleural fluid protein / Serum protein < 0.5 → Transudate

So, pleural fluid protein test serum protein test-এর সাথে মিলিয়ে সবচেয়ে accurate result দেয়।

How is Pleural Fluid Collected?

Pleural fluid সংগ্রহ করা হয় Thoracentesis procedure দ্বারা।

✔ Local anesthesia
✔ Ultrasound guided (most cases)
✔ Safe and quick procedure

Collected fluid laboratory-তে পাঠানো হয় protein analysis-এর জন্য।

Is Any Preparation Needed?

Generally,

• Fasting প্রয়োজন নেই

• Blood thinners নিলে doctor-কে জানাতে হবে

• Previous reports helpful

Procedure-এর আগে X-ray বা USG করা হতে পারে।

Interpretation of Pleural Fluid Protein Report

🔹 High Protein Level Indicates:

• Tubercular pleural effusion

• Malignant pleural effusion

• Parapneumonic effusion

🔹 Low Protein Level Indicates:

• Heart failure related effusion

• Liver or kidney disease

👉 Always remember: single test is never final.

Limitations of the Test

• Cytology
