CSF FLUID FOR Z.N STAIN (AFB)

CSF Fluid for Z.N Stain (AFB) একটি গুরুত্বপূর্ণ laboratory test যা Cerebrospinal Fluid (CSF)-এ Mycobacterium tuberculosis (TB bacteria) শনাক্ত করার জন্য করা হয়।

This test is mainly used to diagnose Tuberculous Meningitis (TB meningitis) — a serious infection of the brain coverings.

🧠 What is CSF?

CSF (Cerebrospinal Fluid) হলো মস্তিষ্ক ও মেরুদণ্ডের চারপাশে থাকা স্বচ্ছ তরল যা brain ও spinal cord-কে সুরক্ষা দেয়।

Normally CSF sterile থাকে। However, যদি TB bacteria প্রবেশ করে, তাহলে meningitis হতে পারে যা life-threatening হতে পারে।

🔬 What is Z.N Stain (AFB)?

Z.N Stain (Ziehl–Neelsen stain) একটি special staining technique যা Acid Fast Bacilli (AFB) সনাক্ত করতে ব্যবহৃত হয়।

TB bacteria-এর cell wall খুব শক্ত এবং waxy nature-এর হওয়ার কারণে সাধারণ stain-এ ধরা পড়ে না।

Therefore, Z.N staining method ব্যবহার করা হয় যাতে microscope-এ লাল রঙের rod-shaped bacteria দেখা যায়।

🦠 Why is CSF AFB Test Done?

Doctor এই test recommend করেন যদি রোগীর মধ্যে থাকে:

• দীর্ঘদিনের জ্বর

• Severe headache

• Neck stiffness

• Vomiting

• Altered consciousness

• Weight loss

• TB history

• Immunocompromised condition

Especially যদি TB infection suspected হয়, তখন এই test অত্যন্ত গুরুত্বপূর্ণ।

⚙️ How the Test is Performed?

1️⃣ Lumbar puncture (spinal tap) করে CSF sample নেওয়া হয়।
2️⃣ Sample slide-এ নিয়ে Z.N stain করা হয়।
3️⃣ Microscope-এ Acid Fast Bacilli খোঁজা হয়।

The procedure is safe when performed by experienced medical professionals.

• Same day or within 24 hours

• However, AFB culture or PCR test বেশি sensitive

Sometimes TB bacteria খুব কম সংখ্যায় থাকে, তাই Z.N stain negative হলেও TB completely rule out করা যায় না।

✅ AFB Not Seen

• TB bacteria detected হয়নি

• However, further tests may be needed

❌ AFB Seen

• Tuberculous meningitis highly suspected

• Immediate anti-TB treatment required

Early treatment শুরু করা অত্যন্ত জরুরি, কারণ untreated TB meningitis গুরুতর brain damage বা মৃত্যু ঘটাতে পারে।
