What is Pericardial Fluid?

Pericardial fluid হলো হৃদপিণ্ডকে ঘিরে থাকা pericardial sac-এর ভেতরে থাকা স্বাভাবিক তরল, যা হৃদপিণ্ডকে সুরক্ষা দেয় এবং friction কমায়।
কিন্তু সংক্রমণ, যক্ষ্মা, ক্যান্সার বা autoimmune রোগের কারণে এই তরল অতিরিক্ত পরিমাণে জমে গেলে তাকে Pericardial Effusion বলা হয়।

What is Z.N Stain (AFB)?

Z.N Stain বা Ziehl-Neelsen Stain হলো একটি বিশেষ staining technique, যা দ্বারা Acid Fast Bacilli (AFB) শনাক্ত করা হয়।
এই ব্যাকটেরিয়া সাধারণ staining-এ ধরা পড়ে না, তাই Z.N stain ব্যবহার করে microscopy-তে TB bacteria দেখা হয়।

👉 AFB positive হলে তা tuberculosis infection-এর শক্ত প্রমাণ হিসেবে ধরা হয়।

Purpose of Pericardial Fluid for Z.N Stain (AFB) Test

এই test করা হয় যখন—

• Pericardial effusion-এর কারণ unclear

• দীর্ঘদিন জ্বর, ওজন কমা, রাতের ঘাম

• TB-র ক্লিনিক্যাল সন্দেহ

• ADA বা imaging suggest TB

• TB endemic area-তে রোগী

এই পরীক্ষার মাধ্যমে শনাক্ত করা যায়—

• Tubercular Pericarditis

• Extrapulmonary Tuberculosis

• Chronic inflammatory pericardial effusion

• TB-related constrictive pericarditis

এই test TB diagnosis-এর একটি গুরুত্বপূর্ণ অংশ হলেও, সাধারণত ADA, Culture, PCR, Imaging-এর সাথে মিলিয়ে final diagnosis করা হয়।

Sample Collection Procedure

1. Cardiologist দ্বারা Pericardiocentesis করা হয়

2. Sterile technique-এ pericardial fluid সংগ্রহ করা হয়

3. Sample pathology laboratory-তে পাঠানো হয়

4. Z.N staining করে microscope-এ AFB খোঁজা হয়

5. Result রিপোর্টে উল্লেখ করা হয় – AFB Positive / Negative

• AFB Positive
  → Tuberculosis confirmed

• AFB Negative
  → TB ruled out নয়, further tests (ADA, Culture, PCR) দরকার

👉 TB-তে AFB negative হলেও clinical correlation অত্যন্ত গুরুত্বপূর্ণ।
